The players wouldn't be able to see each other, but could see one another on the global map. The co-ordination of identical a-life would be too intense to do.

It's just a matter of sending equipment info. Also it'd require rewriting parts of the engine, but imagine one server for each map, every one of them simulating NPCs for just that map and when they're transferred, i.e. they walk to another map, they send the information of that NPC to another server, for it to simulate.
