The Thorian was like the Zerg on steroids. But with plants instead of organic buildings. The background sound effects there were pretty creepy, plus the zombies were ridiculously annoying.
To me it was just one big blubbering vagina
I actually wanted to know more about it because it was so old and could tell me so many things about the protheans and reapers
Holy SHIT, that Yurei man.
That game's practically a screamer
Fuck I was not prepared for that, makes me think why the hell he removed it?

Shotgun the zombies, get a teammate to hold that chick in the air, keep sniping or something.

I'd recommend you don't google this game.
One of the items in her inventory was poop hair....
Somehow I didn't think they were scary at all, and I'm a person who gets scared easily
Same here! When I first played STALKER I quit at the first sewer because I was scared to death. Then, I played it a couple weeks ago and got past X-18 without any fear at all.
It was really only annoying because I couldn't figure out how to stop the boxes from hitting me....
The bad karma sound from fallout 3 scared me when i first heard it
Any of you played Resistance:Fall of Man?
While playing at Conversion Center level, one of the Menials jumps at you from nowhere.
Gives me a jump.

For some reason I had a whole ton of issues with ME1 on my PC, on of them being that the sounds would cut out in areas with a lot of different sounds. Sadly this was one of them.
Nightmare House 2. All of it. Without exception. Especially if [sp] You leave without killing the guy. [/sp]
I realized that I don't really play horror games because even the corridors in Amnesia: the Dark Descent scared the shit out of me. Though the watermonster was not intimidating at all.
Try the monsters that you can actually see, they're quite intimidating.
When I was about 10 years old I played a lot of Star Wars: Dark Forces, I can't remember if I actually beat it, but I certainly can remember this FUCKER. The 'Dianoga'.

I'm not sure why this guy scared me, maybe because he was usually found in sewage water, dark tunnels, or wide open catacombs with limited visibility, it's probably the fact that he emerges from completely opaque water and EATS YOUR FACE. This guy gave me nightmares as a kid.
And the above picture isn't even the scariest part, look at its sprite sheet.

I really got nostalgic chills looking at its attack frame. I remember it so vividly.

That thing's a dianoga, which also appeared in Episode IV. They like to munch trash, so everybody jams one in their trash compacters.
Aye, I remember that thing in the first movie. Damn creepy fucker.
Whatever you want to call these. These creeped me out to no end.

Valtiel is what happens when you combine Pyramid Head with a guardian angel. Basically, he's a...hell angel? Anyway, he's apparently the manifestation of fear of violation (read: rape), or some such. He's also apparently a canonical explanation for game over screens in Silent Hill 3 - he revives you when you die.
And then there's this:

Woe to those who accidentally walk into this room (which you undoubtedly will on your first playthrough). Note that the door magically unlocks as soon as your reflection stops moving, but it's kind of hard to notice that when you're busy freaking out over the creepy red stuff.
Different game, same series:

This is the battle music for a scenario in Silent Hill 2. Imagine walking into a room after it's been quiet for a good while and being hit in the face by this wall of sound. I'm pretty sure it's worse than the usual radio squeals...
Also, perhaps a semi-palette cleanser - a typical player's experience in Silent Hill 2 in a room designed to play on a player's fears of big empty areas/being attacked from behind:

The unlit rooms/tunnels in HL2:EP1 freaked me out a lot. Even though there was no sound OR zombies in most of them, you just felt like you really didn't want to be in there.. plus it was ridiculously hard to think when you can only see a small fraction of what is straight ahead with the flashlight - so it took me like 10 minutes to figure out how to get a shotgun inside an enclosed metal cage..
also, the huge antlion soldier in episode 2 freaked the shit out of me, once again - not because if was giant, but because it was unkillable and ran ridiculously fast..
Oh and this:
prepare to shit bricks

Playing Half-Life 2 early in the morning, waiting for the Melatonin to kick in.
Hear a Poison Zombie moan.
F
THAT

Posion zombies scare the ever living shit out of me.
To make matters worse, I always play on hard and the posion zombies take an enormous ammount of damage to kill. Whenever I heard one, I would jump out of my seat and frantically start shooting RPGs in it's general direction.

Bikers are pretty scary.
That level in Psi-ops, it had this woman, she could talk to you in your mind and one part is just a maze where she plays with you and creates hallucinations of the characters, not to mention in her bossfight, she turns into a fucking monster with three snake heads. here's a video: http://www.youtube.com/watch?v=hKf4-p4Lvqk&feature=related

Nah man, that's just Charles. No need to be scared!
* Silent Hill 3 stuff *
I rarely find games actually scary or creepy, so far only Project Zero/Fatal Frame does that to me. Although when I played Silent Hill 3 with a female friend of mine, I have to say that's also a pretty creepy game. Haven't been to that room yet, though.
Although, I played the Silent Hill 4 demo on PC and that was just not creepy at all. Maybe because I was too busy complaining over the controls and such. :v:
"Lucy" from Project Eden was fairly creepy.
playing limbo for the first time, "oh, what a pretty butterfly!"

I always thought Pyramid Head as the resident lawman of Silent Hill, who torments those sent there for their sins, until they can prove themselves to be better people, or die trying. Not necessarily benevolent, but not a wicked psychotic demon either.
That damn Voodoo Lady in Monkey Island.
