And for content, the bonus levels of Medal of Honour Underground. Some castle or something, bipedal dogs, knights with swords and the Panzerknackers. Scared the fuck out of me, never finished the level.

One of the few recent games that scared me more with atmosphere than jump out scares. Starting off not having any weapon but finding those notes and recordings that implied something really nasty could be right around the corner. Then when you finally get a weapon its that crappy revolver that barely does any damage and you need to fight those zombie things that just don't like to die. Too bad the later sections of the game were such a let down.

Killing Floor, when I was standing in a hallway fighting off a big horde. I turned around to check for specimens, and, seeing none, I turn around to continue fighting off the crowd. When I finish killing them, I turn around to get to the trader before my teammates kill the last one. I wheel around, and there's a monkey man RIGHT THERE. Scared the crap out of me.
I have yet to finish Bioshock 2 because I get scared just walking. It's the creepiest game ever.
A lesser known example, but in the game Soulbringer there are these things called Raals, whom are hooded man-demons that roam around certain levels in the game. They were very scary when I played at 6-8 years old because the only way to kill them was to take down their health and use a stake to make sure they didn't respawn. However, no stake, and they'd be back.

Bioshock 2 was a walk in the park compared to the first. I didn't think it was scary at all. because you lumber around as a bid daddy, destroying everything in your wake. Single Splicers don't stand a chance against you, and you make more noise than a freight train. What I found scary about Bioshock 1 was your vulnerability and the thought of being an ordinary man, alone and trapped deep under water with no chance to escape, with the onlt option being deeper down the rabbithole. In the second, you know Rapture. You were built there and it's your "home." The ocean holds no threat anymore, because you are in a diver's suit and atop of that you're pretty overpowered. All in all, I think Bioshock 1 was a lot scarier than the second, with I didn't find scary at all, and more action-oriented. Maybe it's just me, but hey, we all have our opinions.

Any of the character portraits from the first 2 fallout games, theirs something about them that look so... unnerving. Also those fucking splicers that pretend to be a corpse and jump up right when you walk close to them.
I'm about to play Silent Hill 3 for the first time.. The Lavender Town remix thing just gave me a headache. :[
Also, is it just me or does low res textures for scary enemies tend to scare you much more then high res? I guess its partly because of the difficulty recognizing.

Played a bit of Silent Hill 3. Got gnawed on by some dogs. Wasn't scared.
