BethSoft's being cagey on what they mean by "significant changes" - architectural changes, for example, like dropping cells? - or are they swapping things in and out within a modular architecture? (When do we get to call it a new engine?)

Just like how Src (source) is derived from Goldsrc, and Goldsrc is derived from the Quake Engine. Doesn't make the source engine the quake engine; just like how Creation being derived from Bethesda's Gamebryo modifications doesn't make the Creation Engine the Gamebryo engine.

I might add that either way, the Creation Engine is a huge improvement from Gamebryo from what we've seen so far.
Good that I happened to be listening to LotR music while seeing those pictures. Can't say I'm too pleased with the Wood Elf but I suspect with the name "Dicktits" they were messing around with the character creation rather then being super-serious with it.

You're playing word games. "Derived" is a term that's easy to throw around, the amount of derivation needed variable: my heavily modded Oblivion is "derived" from Oblivion's own Gamebryo, since it augments the graphics stack (OGE), overhauls AI and leveling (OOO), and significantly overhauls weather (All Natural).
Valve has proven that Source significantly overhauls GoldSrc with their papers and the SDK wiki - not that they need to, nor does BethSoft except when I have to back up a performance claim - they've swapped the netcode in and out, added a physics engine ("overhauled" to the point that they keep QPhysics in for players), and, yes, replaced rendering code.
BethSoft have talked about features. I'm very cautious about saying "when", considering this is BethSoft, who went and took Gamebryo for Morrowind and came back with Gamebryo for Oblivion, derived from each other, yet acknowledged to be the same in name.

Also, it takes [I]one week[/I] to generate the distant LOD for the terrain. That is just pathetic considering MGE can generate all the lods for terrain, statics and textures in about 5 seconds

easily modded though, so it isn't too huge a deal - I did it with Oblivion and Fallout
although for some reason texture mods don't seem to be working for me right now, might have to run archive invalidation

Well since you don't know, why dont you default to the people who made the Creation Engine; people who obviously know more than you about the given subject.
Every time some guy feels he needs to refer to Creation as Gamebryo, it does nothing more than spread more rumors among people who are more casual observers of the series. Now you've got troves of people going around "hurr durr creation is gamebryo" even though they have no fucking conception of what they are talking about. So do everybody a massive favor and stop referring to it incorrectly, and save everybody the time and confusion.
