I got trouble assembling the lander, the descend fuel tank gets bugged and won't connect.

At an altitude like this, your orbit is bound to decay due to atmospheric friction. You should get to at least 40km.
I got to the moon, entered a nice orbit, slowed down to release the LEM, engaged the descent thruster...
...and accidentally separated the whole lander module.
Still managed to land it using the ascent stage, but it exploded upon impact so they're now stuck on the moon

Is that the order you connected in?
Watch out for some of the decoupler points, they're tight and can sometimes lead to you picking the wrong one.
If neither of those is the problem, show us a picture.

You need to get down to less than 5m/s in order to land safely.

Almost.
Well I wasn't able to get a really circular orbit BUT my perpiapsis was 660 meters and my apoapsis was 2600 meters. I orbited once with 2600 meters - 1000 meters, then I lowered my orbit to the 2600 - 660. I did 3 / 4 of an orbit and then KSP said it was out of heap memory :(
Will have a video soon. If I knew what the highest thing on mun was I'd be more ballsy about lowering my orbit
Helpful Hint is to just shoot for past the moon's orbit. Just get your trajectory well past the moon's orbit line. You can ALWAYS do a retrograde or prograde burn mid flight to speed up or slow down to catch the moon
The closer you are, the more fuel it will take

Where are you trying to attach it?
Easy way to get to the Moon:
Boost straight up at launch to at least 80km.
Take a heading of 90 degrees with a pitch angle of 35-40 degrees.
Once you jettison your boosters, burn those engines until your orbit meets with the Moon's orbit.
Then just wait until the Moon catches up with you and you get caught in it's gravity. Be ready to burn retrograde beforehand.

open Settings.cfg in notepad and add this:
TUTORIALS_EDITOR_ENABLE = False
TUTORIALS_FLIGHT_ENABLE = False
