Here's what I've found out as I played the game:
The status ailments are POWERFUL. (Spider, Mom's Contact, Common cold. Might be a few others)
I believe the pills are tiered. Unless it's a gold or blue pill, I wouldn't use them unless you have lot's of health.
1. Golden pill: Most likely to be beneficial. BOMBSAREKEY BOMBSAREKEY FULLHEALTH BOMBSAREKEY BOMBSAREKEY BOMBSAREKEY
2. Blue and white pill: likely to give buffs.
3: Red and white pill: Middle ground, can be helpful/harmful
4. Red and white pill with red lines: Most likely to be harmful. Lowest chance to be helpful.
Tarots are meant to be kept until needed. Damage bonus granting ones are to be kept until bosses. Use the ones that give items.
I've also learned what items would be beneficial on starting, and what ones aren't. Yum Heart, Book of Revelations, and the Bible are all keepers, however. If one gives an item on use, use it then get back your other item.
Don't abuse the devil shops, Get one item at most each visit or you WILL pay for it. Greed will definitely work against you here.
Pride is a shitty boss with shitty drops. Not too hard tho.
Greed is grand early on, especially if you get a steam sale. CHEAP ITEMS YES. Be warned that if you get hit by his shots you will lose one coin permanently and drop 3.
Blow up the machines, vendors, gamblers, and beggar if you have no coins. Use them if you have lots of coins. The gambler (middle of the arcade) will give you skatole, it makes flies friendly.
Blow up blue tinted rocks, they contain goodies.
Save at least one bomb at all times. The game isn't afraid of screwing you over.
Chocolate milk + anything is OP. Especially with Trinity or Technology. You will no longer need to fire rapidly. You need to shoot only ONCE.
Pierce is very useful if you can get it. It renders anything with a shield to a mass of gibs.
I'd add more but I haven't played in a while. And yes, I'd say this game has rogue-like elements, I've been learning what to do, what items benefit me at certain points and what will harm me early.
But, opinions.

Devil shops are as useful as you are skilled. If you're shit at the game stay the fuck away from them.
If you can clear most levels with one or two hits at max, abuse the shit out of them.
Also, if you are at one heart, always buy the cat upgrade. You can sacrifice one life to get the other item from a devil shop if it uses normal hearts.

I wouldn't be even slightly surprised if the game was programmed not to give you what you need. I do a floor without losing any health, and all I get is hearts. I get 50 coins but no keys the entire game. I'm on half a heart and clearing a floor without taking damage gets me a bunch of bombs and keys. Etc.
I've been trying to do a no-damage run. I've beat every level individually without taking damage except the Depths, and it's pissing me off. Fucking Monstro II.
There is sort of a glitch you can do on Monstro II.
When he first cast his laser, get on the other side of him. When he finishes casting it he will start to charge it again facing the other way.
Rinse and repeat.
It has never failed for me.

Got to caves 2 and then died. Past floors: a million hearts when I"m at full HP, no bombs or keys. Now i'm about to die, gives me nothing but bombs and keys.
Found Money = Power in the first treasure room. Then found technology and a dollar + later on a wire hanger.
Max damage max tear lasers are fucking awesome.
