Nah, I think you just need to beat the boss on the last floor. You can skip the first one.
Shovel first stage, emperor card 2nd stage. That's the easiest way to do it, but yeah you just gotta get lucky.

Fetus in a Jar is a horrible item! How do I kill enemies that run after me with it? I accidently killed myself instead just a minute ago >.<
It's one of the most OP items, combined with Mr Mega and Bobby-Bomb, it's about as useful as Mom's Knife.

You just can't play with it.
It's one of the most powerful items in the game.
And it also gives you infinite bombs.

2 Horseman Heads and 4 Eye leeches are a nightmare.
Got parasite+triple shot+inner eye on one of my last playthroughs. Dear god that was good item combo.

Lead them along a wall (through a chokepoint if possible) and shoot your bomb into the wall. Time it so the explosion happens once they reach that same point.
Alternatively, just drop regular bombs as you run around, or drop one when they swarm all over you, kamikaze-style. You might lose some hearts this way, but if it's a room like that one with narrow walkways and a bunch of worms that chase you, it may be the only way to escape.
Hmm, in the last 3 days I've played one game a day and won every time. Only one more and I've got my 10th win :D
sadly I've only unlocked 43 items, so I still have a looooong way to go.

Try to get a familiar as soon as you can, Little Steve being the best.
Walk away after trowing a bomb, you don't want to get back there, getting hit by a regular enemy is half a heart of damage, but if you get into your bomb's damage radius, then it's 1 full heart !
And abuse of collisions as much as you can. Take example of [URL="http://www.youtube.com/watch?feature=player_detailpage&v=i2cN4DkEQEE#t=413s"]this[/URL], and notice how he manage to kill the last Fly.

Protip: To get golden god you do not need to do the Mama's boy, spelunker boy etc, achievments. You Just need to get all the items, get every charcter and beat sheol with them.

Little Steve is especially good with Fetus in a Jar because it's homing shots.
Little Gish beats it in every other kind of situation tho.
Little gish is my favorite, he makes dominating bosses a cake walk. Especially satan.
