Theory : [sp]Hisao actually died at the end of Lilly’s route. The hospital lights were him entering heaven, after dying while chasing after Lilly. The reason everything is perfect is because it’s what Hisao wanted in a perfect world - heaven. This explains why Akira did not break up with her boyfriend. Lilly never had a perfect ending. Hisao did.
Lilly flew away on the plane while Hisao passed away from a heart attack. Deny this all you want but you now notice that everything turned perfect suddenly during the epilogue.[/sp]
