Misha and Kenji are the only good part about the Shizune route.
I feel incredibly sorry for Misha, and I think it sucks that I care more about Misha than Shizune in Shizune's arc.
They are, even though I don't read a lot since I don't want the arcs spoiled.

1. Rewrite the Hanako Arc to make it take place over a longer period of time with a less abrupt ending and less frustrating compared to how she evolves in Lilly's arc.
2. Misha Arc (Can be easily replaced with Shizune's arc which is by far the worst of them imo).
3. Miki Arc.
4. Kenji+Nurse+Muto Arc (I think that could be REALLY interesting).
And out of your three it would propably be Misha's. I kind of misread your post so just consider the stuff on top of what I would change in general.
It's still a good game though and I don't think it needs change.
