Aside from the gore, I think you're wrong. Like, there's a lot of blood but that scene in Heavy Rain was pretty tame.

I didn't cringe when it failed, that was fine. I cringed when it succeeded.
I couldn't take heavy rain seriously for some reason. In my first playthrough I did bad on purpose just because I hated that one guys kid. I also made sure that one guy had as much drugs as possible. Also fuck this little shit.
