I know most games can't get away with this, but I like Space Marine's health system. Shield, no regen, melee to get health back.

Only place I can think of where health regen was done pretty well in a 'realistic' environment would have to be a fairly early example, The Getaway.
You could get badly shot up and your health was indicated by how hideously blood stained your shirt was getting, but if you stood beside a wall you would lean on it for a while (30 seconds maybe?) and regenerate health. It meant you had to be well away from any enemies because you were practically immobilised while you rested.

Sure, it's unrealistic, but that doesn't mean it's automatically stupid.
No game is realistic when you think about it. In TF2, Soldiers fire rockets at their feet to propel themselves, medics have magical healing guns. And how is going around picking up health kits any more realistic? "oh let me just pick up this first aid kit and use my expertise medical training to heal all my bullet wounds and burns".

Maybe TF2 isn't the best example? Operation Flashpoint 2 is the most realistic game I've seen, and even that uses the regenerating health+med-kit model.

Though, to be fair, the player is meant to be an officer in the military, and you do need to go through all the individual parts of the med-kit and use them for appropriate wounds, i.e. sutures for cuts, splints for broken bones, painkillers applied liberally, etc.

It didn't feel very good to me.
I'd say MGS3 Snake Eater had the most realistic healing system, hands down. It would be hard to implement into any other game though, unless medkits contained random components for healing yourself.

You can get it for $27 I think? On intkeys.com, I bought mine from there today. Converts to an awesome £17 for me, basically half price compared to the £30 price point on steam or at retail.

In Deus Ex you have a module that re-creates destroyed or damaged tissue. Biofoam is a temporary filler that just stops you from beeding out until they can get replacement organs in you. Medigel in ME is just magic jell that fixes everything somehow.
None of that is in this game so no health regen

recall 3-4 health blocks and aslong as you didn't empty one fully it would regenerate otherwise you would have to find a medkit
