V should have Empire Building system from VCS. That's what R* should focus on, because honestly, even though the gameplay will be more refined and the game will be much better on technical side, V will never be as good at pure sandbox mayhem as SR3. I'd love if they adopted some of things from The Godfather II too, building your own gang especially. Other than that, just take the whole concept from VCS and expand on it. There should be many different rackets - drug dealing, illegal gambling, robbery, loan sharking and so on. And side missions tied to them should also be more interesting than those from VCS. Buildings should be upgradeable, and I think it would be cool if they gave certain bonuses based on a location they're set in. Of couse, you could build anything everywhere, but for example if you build a smuggling building in the docks, you would get some bonus for that, because if makes more sense to place it there than in the middle of Downtown. Also, certain perks would be nice in addition to money. Some free stuff or unique stuff depending on what level you reach in certain building's side missions.
We don't even know shit about V yet besides from the trailer, so you can't even compare it Third in the first place...

You would probably have to google it but if I recall, they had the usual money problems, that lawsuit everyone heard about (a rapper claimed they used his likeness in looks and kind of with the voice) and I'm not quite sure but I think the actor was getting a little pissed with them for some reason.
Niko is still in liberty city doesn't make much sense why he would be in LS, most likely you will work for CJ.
But i dunno what's rockstar's intentions I've heard claude speed would be in some sort of cryo freeze so that's why you never meet him in gta 4 even though you can wear his clothes.
Oh and people are saying the black dude in the trailer would be tommy vercetty well that doesn't make sense either since he's super rich in vice city what would he be doing in LS.
