No, but it's usually a good idea to keep it installed anyway in case you need to reinstall TTW.

It's not hard, just tedious since the only thing the difficulty setting changes is the enemy health modifier (might affect the damage dealt to the player too, not sure since I always just play on Normal/Easy with J.E. Sawyer's Mod)

That's because in FO1 it all comes down to your character's stats, not just the players ability. FO3 on the other hand can be done like any normal shooter, and for some reason VATS only gives 10% of damage during it.

I think the hard mode should be the Hardcore mode on NV

Well that's likely because the Bethesda Fallouts and the Interplay/Black Isle Fallouts are two pretty different kinds of games when you think about it; the older Fallouts were turn-based and the combat depended more on luck of the dice than aiming, whereas Fallout 3 and New Vegas are sort of like the Fallout universe adapted to a hybrid of first-person shooter and Elder Scrolls-style open world, meaning it is easier to hit your target with skill than to roll the dice.
There're likely other factors in the equation, since this is the 21st century and things in the mainstream are trying to be more accessible, but with initial glances my judgement is that the transition from top-down turn-based RPG to open world FPSRPG helped tone down the difficulty of combat, and in doing so probably made it fairer since you weren't at the mercy of Nuffle as the turns progressed and you missed many shots whilst your enemy had superior fortunes.
Nobody's mentioned Victoria Watts. It's especially creepy when you're in a subway downtown filled with nothing but raider corpses and she runs up to you to tell you what a scumbag you are. Why? Because you picked up a holotape once and promptly forgot about it.

Mhm, and it's not a particularly challenging shooter either. Anyone with a basic understanding of WSADM1 shooter gameplay can free-aim their way through Fallout 3 on VHard without much trouble. Especially once they get not-broken weapons.
Heck, I can do it, and my idea of a good round of TF2 is a 0.5 KDR.
For some reason, the game will randomly refuse to let me use iron sights until I restart the game. At this point, it's beginning to not be worth the irritation.

Difficulties the the gamebryo/bethbryo games do NOT affect health stats. They affect damage modifiers, making you do shit damage at the highest difficulty (25% of usual damage) while the enemy does more damage (175%)

So my usual hit and run stealth approach was completely useless in this one AWOP area filled with feral ghouls and crazed ghouls (who were quite capable of one or two-hitting me through my reinforced metal armor). Went back home to fetch a minigun Mk II and the 2000 or so rounds for it I had been saving up; it tore the shit right out of them. My companions proved remarkably useful in covering me while I reloaded.
Fuck stealth from this point onwards, i'm just going to let loose with all the big guns i've been saving ammo for.

Also found what I think was the cause of the previous bug (and a few others); I had "sparks" in my inventory. Somehow i'd managed to pick up one of EVE's energy weapon kill effects. Fuck knows how.
