Honestly, if you want more difficulty, I'd look for mods that actually make it harder without making it bullet-spongey. You're not going to find much challenge increasing the difficulty, you'll only make it frustrating.

If you don't mind throwing balance out the window, installing realistic damage mods does just this. still bullshit sometimes, but hard as hell without feeling unfair.

Speaking of mods, there was one that made you automatically pick up ammo when you walk over gun toting corpses. Is there one for Fallout 3? I have an other mod that increases the amount of enemies. It gets real irritating having to pick every corpse clean one after another with the probability of missing some.
Are there any mods for New Vegas that actually make guns work...well like guns. Cause the fact that I'm missing a dude 5 feet infront of me with a full condition 357 magnum whilst crouched is pissing me off. Not to mention for some reason bullets seem to leave the damn barrel at 45 degree angles. I understand recoil increasing the more I fire. But this is just dumb.

There are some mods that remove gun based bullet spread but still keep in skill-based weapon sway.

I hope atleast into next Fallout game they will make so that alcohol and drugs have a visual effect as well. Seems odd that player can chuck down several bottles of whiskey and vodka and Not have any effect.
Also is there a way to appease the fiends so that they dont attack you right away? I am thinking of making drug/alcohol addict character next. I know that there is quest about fiends that great khans give you, but i dont know more about it.

Fiends be cray-cray. There was dialogue for the bounty quest Fiends recorded but not used. Other than modding, no there's no way to make them non-hostile to you.

The only reason he didn't save her is because he has a fear of Radroaches, you can convince him against this with a speech check.

Doesn't FO3 use Oblivion's engine? I know that things like spells and enchantments are left unaltered in the code (and the enchantments are even used to confer item bonuses) but I think bows and stuff might not work.

Honestly.. I fucking agree so bad. Oblivion with FNV and FO3 would be epic. I can imagine the battle of Kvatch now with the Legion and the Champion of Kvatch armed with plasma rifles murdering deathclaws and the daedra in an ultimate showdown. But only if Butch is the Champion's follower.
I don't mean to poke a hole into everyone's balloon, but there's a few things that would need to happen for a mod to link Oblivion to Fallout:
a) you'd need to rerig a lot of Oblivion's armors back to Fallout's skeleton. Just because they use the same engine doesn't mean they use the same skeleton, and you'd have to update the .nif to work on Fallout's engine because it uses an outdated format of the .nif file. You'd probably also need to recombine the smaller armor pieces into one larger armor piece for it to be more compatible to Fallout, but possibly not if you redid the original underwear model in Fallout to render in pieces allowing for piece by piece armor to work like it did in Oblivion.
b) you'd have to rerig the different body part models listed in each race back to fallout's skeleton again, and once again redo the .nif file. The FaceGen might also be outdated too, so you'd have to fix that
c) you'd have to find a way to port all of the animations that are exclusive to Oblivion (e.g. spellmaking, 2-handed sword animations) back into Fallout. You could probably get away with using default Fallout animations for other 2-handed weapons (e.g. the Warhammer) and 1-handed stuff (e.g. shortswords), but some things don't really have a Fallout equal (spells, mostly)
d) thinking about it now, you'd have to update ALL of the .nifs from Oblivion to Fallout's new nif format
e) you'd probably have to redo some of the weather that Oblivion has so it works with Fallout, but I don't really know the technical details behind this one
f) massive differences between dialog (for example, Fallout always poses sentences as topics, while Oblivion only does this sometimes and brings up only a "topic" (e.g. kvatch, rumors, etc). You also have the disposition system which is completely gone in Fallout
This isn't even including possible issues with quests, scripts that might need redoing, other systems that are in Oblivion and not Fallout and vice versa, plus about a million more things that I don't really understand or even know about at all that'd probably break. You'd be taking on a fuck-ton of work, including having to find a way to mass-translate .nif files from Oblivion to Fallout format without any issues arising and I dunno, you'd need to be a helluva dedicated just to do that and it might not even be completely possible
