Alright, I guess I owe you an explanation for this. :v:
I'm basically working on a module that is automatically loaded by the game and allows you to run custom Lua scripts, a bit like GMod I guess. I don't want to talk about it too much because it's a very early stage right now and I wouldn't want to get your hopes up for nothing. But if I can pull this off, the possibilities are going to be endless. I'm mostly hoping to find out how to add new items to the game, although that's going to be rather tricky due to the way they are internally stored.
As for the second Isaac, it's a very simple change and to be honest, I'm surprised it worked so well. Both Isaacs are almost completely independent from each other, they have their own stats, items, and even coins, bombs and keys, although the second Isaac cannot open doors, only chests (and of course there's the problem of not being able to see what the second player has).
Familiars only follow the first player, but that was to be expected, I'm hoping to find a way around that.
Both players can have a different spacebar item, but again you can't see what the second player has so it's a bit tricky to manage.
Trinkets work a bit oddly for some reason, you can't both have different trinkets because when any of the players picks up a trinket, both players obtain it. If you already had a trinket, you will both drop your current trinket, which, amusingly enough, allows you to endlessly duplicate your trinkets.
And it's so much more fun than the current coop mode. It feels somewhat balanced because even though you have double the firepower, you don't share your items, so you have to decide who takes what. As a result, instead of having one powerful Isaac and a lesser clone of himself, you have two less powerful Isaacs with completely different abilities, which means that both players need to work together and none of them will ever feel like a dead weight.
So yeah right now there are a lot of issues that needs to be fixed (which was to be expected), but I'll try to release something once it's stable enough.

I've never played normal once since getting the game. well, outside of the challenges of course.

Oh cool, I got bucket of lard and became able to stomp stones etc. so I decided to stomp the 2 skulls outside the item-room and got Ghost Baby AND Dry Baby from them. No luck involved.

Every time I decide to do that, all of a sudden every mushroom starts to fart, and every single skull hides the skull-dudes.
Don't even try to do that in the womb.
it's still worth popping the shrooms cause they only do half heart damage and mushroom items are really good, and the skulls don't do contact damage so you can break all of those for free

it's not really any harder you just need to manage your pickups a bit better

I know, I completed 2 hard runs with Isaac and Azazel before, they are just tedious because you need to get real lucky with the items and drops you get

Its hard the way Fallout newvegas Hardcore mode isnt really hardcore, it just add things to annoy you.
I just had three runs. First I was getting very nice items but then I entered a dice room and got Dr Fetus with bunch of babies. Run ruined (or maybe I just don't know how to use bomb projectiles).
Second was going poorly from the start with horrible items so I restarted.
Third time's the charm. Great items (thank you based dark bum). [sp]I got both options to go in Sheol or Cathedral. I chose Sheol first. Satan was alright until the third stage, I barely killed him. Finally unlocked Phil Fish.[/sp]

Caves 1 boss: Pestilence
Caves 2 boss: double Duke of flies (?)
Depths 1 boss: Gurdy (?)
Womb 1 boss: Death
So is there something I should be aware of or Book of revelation DOESNT make sure you get a horseman boss?
if you ever get conquest or a double trouble boss instead of a horseman then you're fucked and can't get meat boy

the second item isn't really the point of the boss rush it's the fact that you get a choice of 4 items which you can take or leave
