I'm going to assume that the model is in fact a modelhack I.E it means that they couldn't have used the alt helmet sicne nobody have ported/ripped it from Warframe, with that in mind i think it's safe to assume that this was either done in SFM or Garry's Mod.

So in general, fusioning. Transmuting is mostly when you have a ridiculous amount of credits and excess mods and want that super low chance at the mod you actually want.

Transmuting I found is a good way to roll for any mod in the game especially if you combine 4 rares like useless warframe ability mods you dont need.
Expect to get a Excalibur slash dash 50% of the time however.
