Most of my headshots are luck, even if I'm aiming at their head the only time I actually get a headshot is when they're running towards me or jumping.

Some guns just seem more prone to getting them as well, like I find that the AM-94 tends to get a fair number of headshots, which I'm guessing is because of the recoil.
