You can start by imagining anything, then you simply create it. If you don't have the skills required to accomplish this, then maybe you should pipe down a bit?

Well 80% of the game is shit, but there are some people and groups that go that extra mile and create extremely high quality sims and make you believe there is dynamic lighting and bump mapping.
10% of the game is furries. 70% are perverted 40 year olds either into bondage or vampirism. 10% are "normal" adults, but still perverts. And the last 10% are camping bots which most of the time are used to sit in chairs where you get 1 linden per hour.
