This is an amazing piece of work and I am thankful that you took the time and effort to write this out.
There seems to be a lot there about sound there, and you are absolutely right that I am terrible at picking up where sounds are coming from and noticing them.
I'm not sure if it's my headphones, my config, or just.. myself, but I have a really difficult time locating sound in 3D, perhaps most noticeable in CSGO (It's not something I really pay attention to in other games.)
I honestly find it very difficult to figure out which direction the enemy are coming from, apart from the most noticeable spots (eg if the terrorists all rush Dust2 B tunnel or catwalk and I am close to those locations at the time.)
Same with gunfire, I don't think "there are shots coming from van" more like.. "there are shots on B site", because that's about as accurately as I seem to be able to hear.
Even if I do manage to figure out where the shots are this makes me second guess myself from "yeah he is definitely there" to "well.. he could be anywhere by now." I dunno how to predict where he'll move to.
I have the config set to use headphones in the audio settings, maybe I should switch this to 5.1 or something?
The zeus was a fuckup, hit the wrong thing by accident because I still use the mouse to buy. Figured I may as well try and use it if they rushed ramp.
I did try to co-ordinate the team a bit with eco's and such but they didn't really listen so I just went along with their force buys, usually.
Some of the times I am not paying attention is because I'm trying to work out a bunch of different things in my head. Where are they? Where's my team? Am I holding this angle at head height? Maybe I guessed wrong and he could actually come from that direction instead (This causes a LOT of the aim swings I made between different angles - I didn't want to hold one angle for definite and then die to behind.)
All this causes me to lose concentration on actually.. aiming and being ready to fire and I have died many times to this now I think about it.
I even did it in the match I just played because I got too distracted by watching the radar and trying to think what the enemy would do, although I managed to correct myself in time to kill the guy.

As long as you are willing to improve you can do no wrong when it comes to losing games. All you have to do is try and understand what went wrong and do your best not to let it happen again. At your rank, all you should be doing right now is paying attention to sounds and being ready to shoot people.
So practice your aim and do your best to keep yourself at head level, which you had no problem doing the majority of that match. You have the basics down, just keep practicing them and then add something else to worry about. Do not try to process every single thing at once, it can really drain. You pick up every little thing as you continue to get better. Don't overwhelm yourself. Just focus on shooting guys and not running into the open.

There are perhaps a few things you can pull from it that may help you with yours, but it's not important to being good. As for sound, my setting is on headphones as well.
don't change it to 5.1 when using stereo headphones, won't help and will likely make things worse

I pop a hard-on when I start ct on maps like nuke because especially the first half it's a huge morale drainer for t. Any team that's reasonably competent will just ignore the score and realise they can bring it back easily but for most pubs it usually fractures their team and they freak out over who to blame for their losses.
And then the enemy team on Nuke destroys you completely both as T's and CT's

That's actually exactly what happened to me, except I started T side on nuke. That's how we were down 12-3. It didn't stop us from crushing it in the second half. So satisfying.

the most hilarious thing is when people start ct side and you end the half 10-5 in their favor and they shittalk and then get crushed because they're ct side warriors
