I believe the shrapnel is real, but it only exists at the smallest size we see it. When it grows, that's his mental image of it.

My guess is [sp]they probably told him the intent of the cloning experiments, not to mention the forced taking of a DNA sample from him, so he left before he really knew just how far Zero and the others went and thus didn't know the results or anything.[/sp]
