The horns in his helmet are most likely his head features (compare with Naga Siren) because he's a merman.
VALVe thought "whatever" and added a bunch of helmets that cut off his "horns" though.

Probability says he's 25 percent fish, but depending on how lucky or unlucky his genetics were he could potentially be anywhere from 50 to 0 percent fish if he inherited all or none of his mother's fish genes, and that's not even accounting for recessive and dominate genes making him look extra fishy or less fishy.
