Technically, you [I]can[/I] get an item-equipped portrait avatar if you actually go ingame and record your hero portrait while wearing the kit, like I did with my Rubick avatar, but it might stutter depending on how you record it, and it's only going to be as high-quality as the ingame portrait is.
My Rubick one took ages to get right as all I was doing to get it was record the game with Fraps, which stuttered really often, and then just cropped it down with virtualdub.

What do you use to make those animated avatars? I'd love to see if I could make some myself, using the cosmetics I have.
If you want an animated avatar with cosmetics, my method isn't going to work. I'm extracting the hero portrait videos from the vpk and converting them to [url=http://blackawps.breakpointservers.com/portraits/raw/]mp4[/url] to import the frames into photoshop. All the videos have no cosmetics. If you want an animated avatar with cosmetics, you're going to have to screen capture it in-game.

Think it was a good time to retire my old creepy avatar. This one does fine.
I used GifCam to make this, but goddamnit it was 200.8 KB in size and the limit was 200.1 so I had to delete a frame.
