CM asks him at one point if he's "still so blue under that armor" (they totally did it) but you can see a bit of his neck which is skin-colored, unless that's something else

My theory is that his horns are the kind of things Naga Siren has on her head and since his entire body is covered in armour he could easily be anything from there.
But how do you explain different custom hats then?

Well his lore does say that something with the connection to the Tidehunter and/or Maelrawn.
Meranths are fishfolk, whom we don't have any visual proof of so far.
We can't tell what meranths look like yet for sure.

Yeah; there's so much lore they could do with it. We now know that the Gem of True Sight is in fact an artifact of Maelrawn the Tentacular, from the latest pages in the book.
