Apparently it only happens when you access your weapon cache.
I'm lucky I beat the game before it happened, or else it might have completely corrupted.

Also, I really love the warden arrival music. It sounds incredibly badass. Kind of makes me wish they had upgraded a bit more later in the game, especially after you finish the story.
