Probably how freddy manages to sneak in when you close both. Or just something Scott forgot to remove before putting it in the game.

Since the door below is open, that could just be Scott hiding the door offscreen while it's not closed. Maybe he just didn't think about the shaking image in death screens and left it there.
Are the doors always open on death? I don't know since I haven't actually played the game, so if they aren't, someone should check if the door is still up there if the door below is closed.

Even if the left door is closed, it is open when Chica does her jumpscare. So, they are always opened when you get caught.

Deep down I wonder why he would have added the door sprite anyway. I mean, the death animations are basically "stop-motion screenshots", the animatronics aren't added to the background as it is before the jumpscare happens.

That's most likely where he left it when rendering the death animations.
