I'm a little iffy on the whole diseases thing. From my experience with NV mods, they're fucking annoying in a bethbyro game. They just end up stacking up debuffs draining your special, max health, skills and in most cases running speed until you can get a completely mundane item which vendors NEVER have. In the absolute worst cases, you get annoying screen blur that makes it impossible to see.
That being said, that's NV mods. Hopefully Beth can make more creative disease consequences than -2 END, -50 Max HP. I'm racking my brain trying to think of what they might be, though.

-5 Strength
-5 Charisma
Interrupting dialogue replaced by shouts of "OH GOD HURRY UP!"

Slower actions like reloading or stimpack use, affecting vats stats or even gun handling like making your aim shake if you have a fever or something. Stuff like that would be interesting.

I bet you it was the underwater content. That shit probably was too much for BGS to develop in the time they had and there is plenty of detail down in the water and cut content to hint at that.

I'd expect them to be similar to Skyrim's.
So, you'll have the stuff like reduced HP/stats, but also things like slower AP regen, harder locks/hacking/pickpocketing, lower damage and so on. There would probably be ones that would impact on needs, so a higher need for sleep/food/drink as well.
It'd be interesting to see if they can do anything with crafting/settlements, but I can't think of anything short of disabling perks (which is unlikely), increasing the settlement food/water/defense requirements, and decreasing happiness.

The second time I went to the glowing sea (in real life, not per playthrough) I didn't die once. Hell, I didn't even get hit once. All I needed was Sneak mode, power armor, and watching where you're going

Radiation storms are, seriously, some of the coolest weather phenomenons in the game. Like misty mornings look cool and whatnot, but those storms are eerily beautiful. I had one once during an assault on Warwick Homestead by a large band of Gunners. The Homestead was getting slaughtered and I called in artillery. All the lasers and explosions and lightning flashes in the hazy green of the storm was such and awesome sight.

click the wall while in the console, hit "getpos z", then enter "setpos z" with a value that's slightly lower than the value returned by getpos. Takes a bit of trial and error but it lets you move stuff into terrain and outside of the settlement boundaries. You can also do this for the two other axes, as well as change the rotation with "getangle" and "setangle" I think. (it's been a while for me so idk if those are the precise commands)

Try making use of the concrete foundations, they can be built sunken into the floor for nice clipping and they also make a neat thick wall that can be walked on top of if you raise them up out of the floor or stack them

You tend to die for a lot of really BS reasons in Fallout 4 as well. Hell even in Fallout 2 on the highest difficulty setting you can sometimes narrowly avoid being overwhelmed if you straight up bolt away from a fight or something, but F4 has a tenancy of hiding really nasty and/or impossible to kill brown enemies amounts big piles of brown, and they move faster than you, etc. Also, enemies with fucking missile launchers. Round a corner and some asshat with a missile launcher point-blanks you for a one shot kill.
Also, [URL="https://www.youtube.com/watch?v=4DnyaDkBWsU"]the number of times I've been killed by touching a car[/URL] is pretty astounding. Sleep saving is just not all that conducive to Bethesda sandbox rpgs that are riddled with bugs and abrupt level scaling, hopefully that one caveat can be disabled, everything else sounds good though.
