38 damage isn't ready. Your time to farm for shards really was before Sen's Fortress.
Going into a boss fight unprepared is more of the "n00b" mistake. Even level 1 challenge players go in with upgraded weapons.
So I got 100% achievements on DS2 SotFS and I feel I can safely narrow down why this game is worse than all the others ( yet still good, mind you )

The game is pathetically easy and when it's hard, it's ridiculously unfair garbage.
There's no pacing whatsoever with difficulty; zones are often ten times harder than their respective bosses and mostly because of one particularly bullshit enemy that has one or multiple of these things; unbreakable poise, infinite combos, extremely delayed attacks, 100% tracking on most/all of their attacks, lingering "shockwave" hitboxes and/or straight up buggy animations.
The only way to properly pace the difficulty would be to fill a zone with Velstadts, Sir Alonnes, Aavas and Darklurkers with the boss being a single Greathammer Drakekeeper. This says a lot about how stupid the game gets.
This is ontop of the fact that world building quality went down, you have no sense of direction and everything feels insanely out of place, like they designed each zone invidividually and haphazardly pieced them together instead of creating one single organic world.
But powerstancing and the simplified upgrade system are all pretty cool so I guess it evens out.

I just met 2 different dragon lancers in the same area. IT'S SPREADING
Refreshing my memory a bit of playing the last DS2 DLC. Why DID the knights in their prisons just vanish when you open the doors? If they could do that all this time, why couldn't they just "poof" out of their cells and be on the spot by the [sp]hole to the Old Chaos and wait for someone to challenge the Ivory King?[/sp]

I always saw it that they didn't have the will to fight the Chaos unless lead by someone brave enough. They probably put themselves in those rooms.

She dropped a set of clothes (Dinky? Dingy? Something like that) and a Black Eye item.
And I had a new Fire Soul to upgrade my vial too...
Oh one more thing. I ended up staying too long idled near the praying parasite dudes in the Demon Ruins (solo'd the witch on my first try, surprised myself there). My character started scratching his head and I grew a giant blob over it, removing my helmet piece. I dunno what it does exactly because I used the acorn item to get rid of it. What would it have done to me if I had kept it?

Also you grew a parasite head, the guys erupt into worms that infect your player. After a while the players head transforms into a parasite, your kick is replaced with a biting worm. The acorn thing is a vermifuge which reverses the effect. It's the only use of an egg vermifuge so there's no reason you shouldn't use it.

The black eye orb is so you can bring justice to the person of that world, and that person is who killed the fire keeper.
Kill the person to reclaim the fire keepers soul, and then [i]don't use it to upgrade your flask.[/i]

No... I only upgrade once with the Fire Soul that was in Undead Parish's Church. I found another one today in Blighttown behind bars guarded by a pack of fire doggies and dart shooters.
I beat up the Queen, rang the bell, got a blob head, frustrated myself with Ceaseless Discharge, turned around and finished my session in Firelink. Found out she was dead, gold dude is gone and I just talked to the serpent with teeth. That's it.
My Flask is at +1 since the beginning.
Seath is by far, my least favorite boss fight in the whole god damn game.

So apparently Amazon Prime members are now getting 20% discounts off newly released games and pre-orders, this including Dark Souls III. However, this only applies to physical copies.

Maybe not, they don't have the will to fight without a brave leader but the will to fight is still there none the less.
What baffles me is that I can do relatively high damage to Smough, is One-Winged angel Ornestein that gives me troubles (he has a resistance against slashing, perhaps?)

Every area must be filled with Drakekeeper Hammers and Maldron the Assassin. Each boss is a colored version of Smelter Demon with different properties and delays.

You can try farming leeches in Blighttown, they drop 1 large titanite shard or 5 green shards, its good for farming both shards and large shards, especially because green shard can be split into 5 titanite shards each using Kingseeker Frampt's feed "titanite x" option.
But of course, first get the Covetous Gold Serpent Ring and get your humanity to 10 before farming like others said.

He's probably got more than enough large shards since they're littered around Sens Fortress. It's the small ones he probably needs, and he can buy them for 800 souls each.
My plan was to farm Anor Londo and buy the shards I need from the giant blacksmith
