A truck or something must have exploded. We saw fragments of vehicle fly through the air and you guys were in the centre of the road so we thought you had bit the dust.

I hope the number of enemy combatants is scaled with the extra amount of players. So we don't get a case where two squad get all the fun and the others just tag along.

I want all the players to have fun, rather than letting everyone play and it being trivial.
Not really sure how you would manage an op for 40 players. Even just spawning enough AI would be a problem.

Only bad thing is that people can die for bullshit reasons and feel cheated for not being able to play anymore. And no, I don't think the mass respawn after an objective would work for a large player-size, unless it was only huge, terrain crossing objectives.
