What are you willing to bet one of these "secret new transformations" is Bob, finally giving players an incentive to pick up his items.
+1 bomb on hit/kill, immunity to explosions (but no healing from them, still need pyromaniac for that), and explode on touch would be fun.
Out of a few extreme scenarios, still not worth it I think.
So it looks like instead of (at least some) shit overriding each other, they can all just combine. That's pretty fucking huge and going to be awesomeeee.

Hopefully having Dr. Fetus synergizing with as much stuff as possible would help it not be shit anymore.
I really hope this applies to Soy Milk too. For a special item it and Dr. Fetus are the items I try to avoid as possible since they're straight downgrades unless you get a few specific synergies.

Serious question, even with spectral tears and the D10, is a lost run even worth continuing if you don't get the holy mantle before the end of floor two?
The only success I've ever had with the lost has been thanks to the mantle, and even then it's still face-rendingly tough to even get to mom, let alone the heart.
I've long since given up on some of the challenges. Ones like head trauma, solar system, and cat got your tongue can fuck right off. I got the powerups from challenges I needed and I beat Lost on hard. I just play for fun and try to get all characters full hard checklists.
I managed to do all the challenges, now I'm waiting for certain items to drop and to get 999 coins into the machine.
Banking coins is probably the most glacially paced challenge in the game, from my experience.
Maybe it's just my luck, but the donation machine never lets me deposit more than 4-5 coins in any run where I have more than that.

I've been told you can farm it with judas (since he always starts with three coins) if you're reeeaaally patient, but I'm not quite that desperate yet.

Another fun idea for a new transformation:
Mama's Boy (obtain three "mom" items)
> Bloody knife appears on the character's front, unable to be fired like mom's knife but still does damage, and inflicts fear

Should be soul hearts exclusively because Mom's Pearl.
Mom's pearl would affect that and change it to soul hearts if you have it, as would the devil tail making them more likely to be black hearts

Here's a list of collectible buffs and nerfs I'd like to see in afterbirth because I have too much time on my hands.

Dead Sea Scrolls: charge time reduced to 1 room
D6: Decreased chance of rolling other active items.
Monster Manual: Spawns 2 random familiars instead of 1
Tammy's head: charge time increased to 2 rooms
[B]Passive items:[/B]
Abel: Fire rate increased
Aries: base 30% chance of preventing contact damage if issac ran into the enemy at full speed
BBF: Can no longer damage Issac
Bob's Brain: damage slightly decreased. Can no longer damage Issac.
Brimstone: slightly increased charge time
Cain's Other Eye: Issac's tear stat increases/decreases apply to the familiar.
Cursed Eye: Teleport effect no longer works in Boss rooms. Tear charge count increased to 8 (up from 4)
Epic Fetus: decreased damage (x20 damage to x15 damage)
Eve's Mascara: Damage increase is x2.2 (up from x2). No longer affects shot speed.
Experimental Treatment: Actually ups all stats like the item description says, then shuffles stats. Number of increases will always be equal to number of decreases.
Guillotine: Head's orbiting speed reduced.
Magic 8 Ball: Increases luck along with shot speed.
Match Book: slightly increases damage along with original effects.
Mom's Coin Purse: All 4 pills will have positive effects.
Mom's Purse: Spawns a random Trinket upon picking this item up.
My Reflection: Range is set to a base x2.5 multiplier.
Pageant Boy: Permanently increases the spawn rate of nickels and dimes.
Scapular: Activation condition buffed from half a hart to full heart when in Womb and farther.
Soy Milk: damage debuff reduced from x.2 to x.33
Tiny planet: x2 tears in addition to current effects
There's Options: Effect carries over to Krampus and angel room fights (spawning an additional angel room item as an option over a key piece.
Whore Of Babylon: Same as Scapular.
The Virus: Major damage buff to poison.
[B]Trinkets:[/B]
Butt penny: Fart now poisons enemies.
Cain's Eye: 25% chance can be raised further by ucks stat.
Cursed Skull: Grants Necronomicon effect when entering a room Issac has teleported out of using Cursed Skull.
Ring worm: Shots are now spectral.
Umbilical Cord: effect lasts until the end of a floor and triggers at 2 Red Hearts or less.
All I want is for them to nerf the Lump in big rooms.
Those pieces of shit are as annoying as Mask of Infamy.

Comments on these, in order:
- Agreed, but the scroll is a useless item no matter how much you buff it, really. Giving Isaac a random extra passive on use for the duration of the room would be nice though.
- D6 should follow the same rules as 4-dot rooms, able to change usable items into passive ones, but passive items will never change into usable ones.
- Rather than changing the number of familiars the manual spawns, the sole familiar spawned should last the rest of the floor. Reward players who can find batteries with multiple familiars, giving a compelling reason to hold onto the manual for more than just novelty.
- I feel this is very controversial, but I don't think tammy's head is necessarily supposed to be a balanced item. It's actually awful without good synergy to base itself on (brimstone and ipecac are the two exceptions), so I think players who get lucky enough to find it with a good synergy should be rewarded.

I played Rebirth again after a long pause, noticed Cain doesn't have Boss rush or ??? completed.
So I played on and, as usual, didn't get any health items of any kind, except or course a pill that took out the other heart I had. But then I managed this:

Aaaaand there we go. I don't usually pre-order, but it's bound to turn out fine, and the price is no skin off my arse either way, since I usually spend more on snack shopping.
And what's more we know what we're getting and have been told and shown plenty of info to make an informed and intelligent decision.

Wait why does the page for Afterbirth say "Mutiplayer" and "Co-op" now? Regular rebirth just says Local Co-op.
