In tf2 arena I was 1vs1, as a scout I was jumping around to dodge other dude's bullets, and I fell to my death and we lost, even worse that dude had only 10hp.
Once when I was playing Battlefield 2142 I was the transport ship pilot and I had 2 other gunners in the ship I was flying really fast and I decided to be really cocky so I tried to fly low to the ground. I flew too low and hit a friendly tank, we all died.

Once I was the last guy on my team and I was hunting down the guy on the enemy team. I found him as he was disarming the bomb with a few seconds to go. I ran up behind him to stab him when the time ran out. The bomb went off killing us both. :doh:
I back peddled in L4D away from a boomer, killed it, got splashed and fell off a ledge (As in the hanging thing)
I once managed to kill myself and my entire team with a grenade in CSS.

It's just a bad choice. Especially when he sees you.
OH! I just remembered I've only finished No Mercy on L4D once because:
The first time I was pulled through the vent at the end and hung off a ledge.
The second time I dived onto the chopper and a Smoker pulled me OUT of it, it took off when I was pulled out.
The third time I was hit THROUGH the chopper by a Tank.
The fourth time I jumped out of the choper to save a downed team mate. I managed to save him but was downed myself, I was Left 4 Dead. (I hate myself for that.)
The fifth time I made it.
The sixth time I slipped off it as I took off.

That's pretty kick ass.
Not necessarily embarrassing, but I always find it hilarious whenever I take down a helicopter or some sort of aircraft in any game and its flaming corpse comes down ontop of me. Or anyone else for that matter.
When they first updated CSS with more than just one skin for each team I spent like a whole map killing my teammates bein like "That bitch doesn't look like me, why you gettin all upset?"

I was pyro on an arena server, playing nucleus, last person on my team, and managed to kill a solidier, a medic, and a scout. The last guy was a pyro who was on the cap and he airblasted me off to my death -_-

Falling to my death repeatedly in every game I own. (fuck you Battlefield, I shouldn't DIE from falling 5 feet).
In Rainbow Six: Vegas 2 for the PC, I was doing Terrorist hunt, and since the enemies are godly with a Spas 12, I strafed away from one. The bullet missed, but I fell off of the ledge, and died from a 10 foot drop.

grenades don't kill you instantly even if they're right at your feet.
well my ingame name for tf2 is Mike.
I was a heavy and a spy was disguised as me.
I saw the spy (as a heavy with my name) and went up to him and jumped and said "Kickass, same name same class".
I turned around to walk away and he killed me.
Honestly I laughed my ass off at how stupid it was :P
Always good not to take yourself too seriously and to be able to laugh at yourself.

That happend to me except replace A10 with a Bycycle
playing rainbow six vegas 2, run into a room with a terrorist standing there with his back to me, I got startled, empty my entire mag (hip firing) at his back, all the bullets miss him, he turns around and shoots me in the face.
Also, another time, was causing a diversion by running around and shooting allot, was about to go hide in a room and pick off terrorists that walked through the door, except I emptied my whole mag, and found out then that you cant open doors while reloading, I became swiss cheese .3 seconds later.
I was playing a 1v1 with a friend on Metal Gear Online, I tried to throw a grenade at him, but it bounced off a wall, blew me off a platform and I fell to my death down a hole to the lower levels of the map.
In CS:S, grenades actually go slightly upward when you've just thrown it. It bounced just at the top of the door, so I boosted it out and jumped, then I fell and got shot up.
I don't know if this has been posted but I charged an arrow with my huntsman (TF2) ready to fuck this pyro up. So i go ahead and shoot and he air blasts it back into my face and i get pinned against the wall with my own arrow.
Some guy chased me off of a ledge in Age of Chivalry.
GTA 4. Freemode, I was driving a NOOSE van with some friends who got 6 stars. We stopped to pick up some random guy who needed help. Next thing I know I'm hearing a beep and my friend screaming "FUCK" before the van blew up killing us all.
Was playing Halo 1, playing on that map with the two halves of a ship, I jumped away from a grenade that was thrown but then fell into the bottomless pit :v:
