Probably in FH2, threw a satchel charge into a room and ended up killing myself and several team-mates.
Hmmm.... I'd have to say it was this one time in TF2 when I attempted the cross-battlements jump on 2-Fort, went a little too high, and died on impact right in front of the enemy sniper. Then later, I was a sniper, and an enemy Demoman did the same to me, only he succeeded and beat me to death with his bottle o' scrumpy.
I got killed by a goblin in Runescape :smug: Still don't know how and why I even played that game.
I had the most awesome headshot in BF:BC, It was from 500 meters range on a sprinting guy, and bullets are slow in BF:BC.
The guy I shot was a teammate. (Still most awesome headshot I've ever, ever made)
Whenever I fly a chopper into a building or a bird in BF2
When I got shot out of my Jet in BF2 by artillery.
Spawn camping tanks in BF:BC with satchels. But some teammates decide to go into the enemy tank.

Whenever I get a 25 damage pounce in L4D that I could dodge easily. But I'm to ego to do that.
Accidently meleeing a witch in L4D when I was last survivor.
Being incapped by a hunter in saveroom (Again last survivor)
Headshotting myself in Dark Messiah multiplayer (Arrows straight up and not moving by accident)
Tf2 with friendly fire, accidently shooting an arrow into the sky with huntsman without noticing, not moving...
Self-owned.
Getting killed by my own sentry gun in Tf2.

TF2 , playing soldier on Badlands blast a pyro of the spire and go for the mid air shot its a crit and the little bastard reflected it straight back at me :(
In Mario Kart DS I once shot a green shell at the wall, which was straight ahead so it bounced back into me.
Also when I'm a Soldier, and I'm hurting myself so my Medic buddy will get his uber up faster, and I accidentally blow myself up.

Well, I decided to give in and have a go at the world of warcraft trial.
Within about 20 minutes, some guy said he'd help me out if I killed some bear or something, so he invited me to his party. What I didn't realize was he'd actually decided to duel me. I asked how to wear some armour, and he said to
"Drag it from your inventory to your character and press enter quickly, cause it's glitched at the moment"
I did so. Then he lead me to a boar, told me to kill it. Needless to say, I died, and he teabagged my naked corpse, whilst shouting "FOR THE HORDE!" and his mate going "VICTORY FOR THE HORDE!"
Then I got bored and started running at people and trying to make it look like I was doing inappropriate things with the taunt commands.
In Halo 3 I threw a sticky .3 secs before someone put down a bubble sheild, the sticky got caught IN the bubble of the bubble shield, and got rebounded out back at me, stuck me, and they called it a suicide.

EVE online was the first MMO I ever played. So what's the first thing I do when I get a good ship? Go into completely unprotected space and get obliterated losing everything because I forgot to fucking clone myself.
Arma 2, back when the 386th was around.
I was piloting a harrier(I think) on a domination map while the rest of the guys got armed and lined up gettin ready to go. I taxied past them trying to feel kind of bad ass. I reached the end of the taxi way and taxied onto the runway(narrowly dodging the C130 they decided they were gonna use instead). As they were marching to the C130, we found out that we did in fact have a A-10 at our disposal. We figured that would be more effective, and I should take that instead. So I continued taxiing down the runway. Not really thinking straight and forgetting I had cycled through my weapons and left my bombs selected, I pulled the trigger. Cue me dying, and the rest of the guys seeing a massive fireball in the middle of the runway.
They decided someone else would take the A-10 instead, and i needed to haul ass to the C-130. Best part is that the harrier left a crater, which barely left enough room for the -130 to take off (it practically scraped it)

That happens to me ALL THE TIME.
On halo 3 TDM on the pit. Creeping along, One of those panles with a brute on it pops up, and scares the shit out of me. I panic, and shoot it. which is a bad idea when standing a foot away and weilding a rocket launcher.

While creating an improvement to my once popular jailbreak map, I placed a barrier and some explosives next to the door to get out of the prision. So being the tester and blowing the concrete "door" open, it flew and smacked me in the face, killing me.
accidentally killed myself with the plasma gun splash damage in Quake LIVE.
Last night in Team Fortress 2 on arena_lumberyard. I was playing with some of mates and my mate Thorny was a pyro and I fired at him with the huntsman. He whipped around and air blasted the arrow and it went straight through my head. I was so ashamed :( But I had maximum respect for Thorny!
