A stealth-game with ultra-realistic AI and actions would be awesome. Trip one guy and make his gun go off, killing the people around him.

I think a mix between Dungeons and Dragons, through a system like Dwarf Fortress would be nice. It would be all for great immersion, as in very good AI and animations, and an environment that is practically completely interactive. As for persistent death: Who says you have to get killed so easily? One major point for perma death is that you fear it. You wouldn't do completely stupid things in the game, making it more of a real world choice. However, for those who played DnD before, they should know that some DMs allow everyone to die. Think of the game like that, so you want to make good choices but you still are pulled towards action. As someone said before, if the world can be created anew when you die, and if you didn't spend too much time grinding on your old characters, having a new one should work just fine.
Maybe if there was realistic leaning.

Personally I think it would be great fun simply because not being afraid of death or personal injury would make the real into unreal. People would cut their hands off and fight with stumps for laughs. Or get horribly burnt so they could run around pretending to be a zombie.
Or they can have a life and play zombie games to begin with.
I can imagine an fps like counterstrike, and during a firefight people piss and/or shit themselves because they didn't remember to empty themselves before the match. Or an fps where players fall asleep etc.
I would make a Quake mod.

They're not ABSOLUTELY realistic but the closer to reality things get, the more fun I find it. You can't say it's fun inhibiting if there's a huge market for the ArmA/OFP series, flight simulators and the like. It's subjective.
Think about this scenario in game: theres people firing i better hit the dirt. * knocks his head on a piece of debris and loses consciousnes* Fuck, im dead? or am i? well a couple of minutes more and il find out.
I've always noticed threads talking about "Game ideas" usually involve cranking up the Realism or Free-Roam to 11, or some other excessively hyperbolic level. People love to say they would kill for the game in the first 2 pages, then reality sits in.

How about instead of "Real world" realistic, we think "Movie" realistic. As in, you don't see Bruce Willis get shot in the face and just stroll around like nothing happened. But you don't see him die from a paper cut either. In movies they can't be riddled with bullets and still walk around like nothing's happened, like games do, but yet they actually have to show some sign of damage in how they proceed after they get shot.

More realistic then insurgency, if you get shot in the leg you fall down, if you get shot in the arm you drop your gun, get shot in the foot your walk speed is fucked up.. Ironsights are realistic, weapon recoils are realistic, you can loosely hold the gun for menuverability or tightly hold the gun for accuracy. It has realistic crouch/walk/prone movements. It's a free mod for HL2 and it's in alpha.
Yeah because you totally know how a gun recoils ingame. Looks like a ARMA spinoff.
