I used to do some cybering on Club Penguin and Blockland. Pretty sure I was 10 or 11 at the time. Should've seen the stuff we came out with on Club Pengiun to get around those chat censors.
really? Minecraft - griefing. Sleepy things in WOT - shooting teammates, going where teammates shots might hit me. Running around a field in a light-armored tank destroyer, forgetting that you shouldn't be here but it is too late. AOE3 - forgetting to build an army until age 3-4 and then getting swarmed.
The Dumbest thing I have ever done in a game was a simple, and childish revenge plot on some man in Minecraft for raiding and killing me. I simply offered him sugar canes and shit, recorded his coordinates, waited until he was offline, and then I set a faction home near his area. Afterwards, I proceeded to raid everything useful in his home with the exception of the chests, and then I crafted a trapped chest. Then, I secretly lined the walls and floors with TNT beneath the actual floors, and wired the Trapped Chest to the stockpile of TNT. Needless to say, when I came back, there was a huge crater reaching bedrock where the base had once stood... Quite stupid of me to do now that I look back at it...
I was on a server I liked in garry's mod and my sister (who was 9 or 10 at the time iirc) got the microphone from me and started talking into it. I thought it would be funny if I let the rest of the server hear it and so I pressed the "make microphone work" button (I can't remember what key it is?) and she started going "Heeeeeeeey." We both thought it was hilarious but the admin of the server got mad at me and muted me.

I played that Half-Life 2 Insurgency mod when it came out and since I have a pretty knee jerk reaction to people jumping around corners at me I pretty much killed my teammates a billion and a half times before just high tailing it out of there and never playing it again.
When I started playing Anarchy Online, I sold a Grid Armor Mk I disk to a vendor because I thought it was useless trash. For those who don't know: Grid Armor is one of the draws of playing the Fixer class. Its a nano program/spell that summons a back armor that cannot be worn with any other armor. It confers a large evasion boost when worn. It comes in 4 levels(technically 5 but I don't believe Mk V actually drops yet), can only be found off enemies and comes in a disk that has to be tradeskilled into a crystal that teaches the skill. Mk I is naturally the weakest version but at the time could fetch 30 million in game money. Having just started out that 30 million would got me a long way, but I had no idea what I had lost.

i somehow managed to follow an enemy for around 2-5 minutes in Red Orchestra: Ostfront 41-45 without realizing he was on the other team i don't think he did, either. we locked eyes while sitting in a foxhole and he gave me this mean glare while checking out my uniform he shot me in the face with his rifle and disconnected

When I first started playing CSS I heard AFK so much I thought it was just a really popular clan for CSS. I tried to find AFK servers and asked people how I can become AFK and didn't learn what it really meant till I started playing GMod.
Back in WoW, during Wrath, my guild was doing Trial of the Crusader for the first time. Before the Anub'arak fight, the floor broke. At the bottom of the pit was a pool of water. I used Path of Frost for some reason. Everyone died from fall damage because of it. I was kicked from the guild. Oh well.
10 years ago in Star Wars Galaxies I pretended to be a girl so that people would buy me things(I was 6-7 at the time). Some dude bought me a Y-Wing, a few speederbikes, and sent me a ton of money.

I accidentally got a quad-teamkill with a rocket during a Day of Defeat Source league match. Since the teams are 6v6, the other team capped out all the flags really quickly. We still won the match, but that one cap out was super embarrassing.
Enabled TNT on my minecraftserver
A couple of years ago, I was in some machinima group that had just started, and we were setting up a scene in HL:DMS. Right after we had finished setting up the scene, my finger slipped and hit the "launch grenade" button, destroying everything we just spent a 6th of an hour setting up. I was kicked from both the server and the group right after :v:
Pretty sure I was banned from every gmod 9 build server that didn't use prop protection. And the ones that did I would build some huge laggy mess of shipping containers and thrusters to wreak havoc.
Here I was, doing framing frame on Overkill with pubs on Payday 2, day one was smooth, day 2 was easy as shit, now here's day three, and I decide to be on charge of the cameras, and while I kept seeing the cameras I got bored and decided to melee the air with a knife, but then, rather than pressing e, I had accidentally pressed 3, which throws a grenade, and it alerts everyone. The other three guys were not happy about this. :v:
a long time ago in halo 3 me and a friend were playing driver. he was driving a mongoose and I was driving a warthog, both full with teammates. both of us turned the same corner in opposite directions at full speed and had a head-on collision, basically killing half our entire team at once
