Arguing with my mother over what I now see were stupid/trivial things.
Getting annoyed over being called to escort my grandmother to the store, only to have the trip delayed an hour because she decides then is THE time to rearrange stuff in the garage.
Not keeping in touch with various people ranging from elementary school to ones I've met in online games.
And because I gotta have one that's stupid compared to the others: letting my mother pick Fish for me as opposed to Prime Rib at my Uncles wedding. Never heard of the prime rib cut at the time. While the fish wasn't bad, tasting a piece from my brothers dish made me wish I had gone with the prime rib.
I decided not to visit my Grandfather at the hospital. He died the following night.

The one day I didn't visit my Grandmother was also the night she passes away, It's a shitty feeling I know.
I called my grandmother on her birthday, later in the afternoon so I wasn't interrupting lunch. Rang, rang, rang, went to answering machine, so I hung up and decided to try again 30 minutes later. When I called back again, no answer, left a cheerful little message wishing her a happy birthday.
Roughly two hours before my first call, she had had a massive stroke that left her in a coma she would be in until she died two weeks later. I was on the other side of the continent and in uni, so I couldn't afford to leave to go to the funeral or see her in the hospital. My other grandmother died in the same year from advanced lung cancer that was detected too late. Again, I wasn't able to pack up and go.
I never got to say goodbye to any of my grandparents. Or go to their funerals.
