In the [b]Last Samurai[/b], Tom Cruise almost suffered a fatal blow to the neck by an actual katana blade. In one of the scenes that involved him riding a mechanical horse (tolduhegay) one of the extras almost swung the katana blade too close and managed to stop the blade an inch away from his jugular.

In [b]Avatar[/b], the Navi are seen using their bows and arrows in a different manor that humans would do. Their firing hand (i.e. the hand holding the arrow plus string) is upside down. James Cameron developed this idea and began teaching the Navi and Avatar cast how to fire bows the regular way, so they'd get an idea of how the bow works. An Olympic Bow and Arrow trainer was hired for this task. After James told the Olympic trainer what he wanted them to do with the bow later(firing with the upside down hand), the trainer scoffed, saying that it was impossible. James then picked up a bow and told the trainer that he could hit a far off bush using the Navi way. And that's what James did, stunning the trainer. The trainer accepted the idea and integrated it with the training. He later admitted that it was a lucky shot.

Mark Hamill, the man who played Luke Skywalker in Star Wars, is also the voice actor for the Joker, starting with Batman: The Animated Series. His last time voicing the Joker will apparently be in Batman: Arkham City. Less known is that he voiced Firelord Ozai in the Nickelodeon animated series Avatar: the Last Airbender. Speaking of Mark Hamill, his son had a cameo in Star Wars Episode 1: The Phantom Menace, as a Naboo Royal Guard.

The Lone Ranger is the great uncle of The Green Hornet. Also the same vat of toxic waste that gave Daredevil his powers is the same vat of toxic waste that created the Teenage Mutant Ninja Turtles. The creators were big fans of Daredevils. In the original TMNT comic the toxic waste bounces off his head and runs down into the sewers.

The errors are there to confuse and disorient the viewers. The entire film is meant to invoke labyrinths and mazes.
If Kubrick would have used the [sp]hedge animals instead of the hedge maze and had the awesome fight between Dick and the lion[/sp], that would be one of the only things to make the movie better. Well, the Overlook Hotel (irony) was supposed to be evil and haunted. [editline]4th August 2011[/editline] But, there are some similar errors I've noticed in Full Metal Jacket that do suggest otherwise. Then again, he did shoot countless takes for many of his shots, to the point where perhaps these mistakes were made by the crew and actors themselves on pick-up shots and days of shooting and Kubrick may not have seen this in the dailies and even if he did, he probably couldn't muster everyone back up a few months later just to get one shot right, when the performace he was satisfied with is already there.
But those are only a few, there are LOADS of continuity errors. A perfectionist wouldn't let that slide. There's a lot of explanations showing how adding all the errors up together can show consistent themes in the 'continuity errors', showing that they might be on purpose. I personally think that they're on purpose and that Kubrick's reputation as a perfectionist would've helped as a smokescreen. Anyway, he's always said that the magic happens in the editing room as no other medium has that stage.
Well, just because he's made errors like that doesn't mean he wasn't a perfectionist. He was probably more of perfectionist in the actor's performance.
While that does sound more likely, you wouldn't think that overworking actors until they fall physically ill for months at a time would help.
All the street names mentioned in [b]The Matrix[/b] are based on real streets and intersections found in Chicago, Illinois.
