The only real time this is possible is when the dosages of anesthetic are fucked up (which is incredibly hard to do) and even then they won't be in any state to determine properly what is happening. I work as an anesthetic technician alongside anesthetists. She's probably paranoid about fat shaming and her mind just conjured it up while she was out.
