Actually some did. There was no "ONE CODE OF CHIVALRY" and numerous versions existed. Some focus on defending the faith and respecting the churches orders. Some dont even mention women, while others do. Others are all about being nice to the bloke you just jabbed a spear in to.

It has to do with making it easier to look at and reducing the chances of the reflective surface of the watch giving your position away.

Because it feels more natural. The inside of your wrist points at your body more often, you can just slightly lift your arm up with your palm facing you to check your time rather than twisting your arm to look at the outside.
Also think Metro 2033/Metro Last Light on the way he holds his watches.

If you're an engineer or something like that, it prevents you from accidentally damaging it. If you're a soldier, it's to not give off glare and give away your position. If you're a nerd it's because you want to look like a soldier or an engineer
