I forget what it was, but there was some browser game when I was a kid, think themed around Wily Wonka candy or some dumb shit where you were in a maze inside a pyramid or something with a monster hunting you which freaked me the fuck out.
Also, TF2's collision. Like, hear me out here. I'm playing a heavy or a pyro and I'm minding my own business walking through my base before I get to the battlefield and all of a sudden I stop momentarily as I turn a corner or I move to the side without my own input as if sliding along something physical. I will fill that fucking room with lead and fire until i know that i either bumped into bad collision or a fucking spy 90% of the time, it's the friggin collision with something on the floor that somehow got in the way
While playing Hot Wheels Turbo Racing on N64 as a kid (like 5 or something) I was driving along the track and saw a cattle skull. It just terrified me, I thew the controller down and ran upstairs to my mother. Man, what kind of kid was I?
The castle from Super Mario 64 in general creeped me out when I was younger, probably because it was barren asides from the Toads (which seemed more like lifeless dolls than anything) and the Boos in the courtyard. Not to mention the almost maze-like design of the place.
I found the PS1 version of Doom to be extraordinarily terrifying at parts as a child. It did go for a considerably more creepy vibe than the original, but not to the extent that some levels I was too scared to continue.

FlatOut 2, when your engine is on fire and you see Malcov catching up. For real, that guy is a wild card.

This one time I was playing Skyrim with some music playing in the background so I couldn't hear anything. I was in a Draugr dungeon and rolled through a set of three swinging axes. I thought I maybe heard something, so I turned around, though not expecting anything because I was sure I killed everything, and a fucking Draugr corpse shoots out of the hallway of swinging axes and scared the piss out of me. I'm going to assume what happened was: the draugr woke up, ran after me into the swinging axes, died while running, and his momentum carried him past my face and scared me.
Pretty much every level in Thief 1/2. Even the non-horror levels had something spooky about it.

I was playing Condemned: Criminal Origins so I was already on edge. It was also the first game I ever played with full body awareness and dynamic shadows. I turned a corner and got jump scared by my own shadow.

I remember the first time I played Elder Scrolls Oblivion back when it was released, I was scared of the Rats and the Zombies in the prolouge for some reason, never made it pass the Tutorial level because of this.

In HL2 there was that one moment where you enter an abandoned house on the highway level. And as you reach the 3rd floor you see that crossbow in the corner of the room. The odd, yet calm beach atmosphere and the sudden earthquake that occurs when you step on the sand freaked me out when I was younger. The same applied to the small beach that you fight the Antlion Guard on. I stood on the rocks knowing that something was going to appear but I just stood there and refused to move for a good while. It doesn't help that a stinger sound plays the moment you see the corpses and the lack of items to walk across the sand on. I don't know if the Knaaren from Rayman 3: Hoodlum Havoc count? There's a ton of people who just saw them as obstacles during the stealth tunnel sections, but the fact that they're invulnerable to all of your attacks and chanted about the gory things they'd do to Rayman if they caught him forced me to stop playing the game for a short while.
Opera House and Asylum levels in Painkiller are kind of unsettling, especially when theres no fights going on. You'd expect it to be scary because one is themed to be a bit unsettling and the other is a goddamn asylum full of what are probably damned souls, but then you're in Painkiller, so you shrug it off. Arsenal Gear in MGS2 was proper weird the first time. The Colonel AI getting bugged was spooking the shit out of me, and the sudden change to the kinds of levels made me believe the game was fucked or something. Plus, I was young as hell. Just how young? It was back in '03, and it was MGS2 on the PC! Also, damn Otacon in MGS1 during the elevator ride in the Comms tower. That closeup and the guards out of nowhere in the elevator spooked me more than actual horror games.

The orc and goblin infested cliff-side wooden places in Dark Messiah. It's not the orcs and goblins that are scary, it's the atmosphere and ambiance.
The beginning of the fuel plant (I think) in Dark Forces 2. After killing a few easy enemies, there was this door leading into a multi-level tower with a couple fuel tanks around. Inside, there were a couple lizard mercenaries with shock rifles. When I was young, I generally play around that beginning area without venturing forth mostly because those guys. After like 10 years, I replayed it just fine but I still felt wary around that area. Shock rifles are fast, painful, and shoots explosive rounds in a confined space with a bunch of explosive barrels around. Arguably the most difficult and mentally enervating area for me.
I thought of another one: this one time I was playing Hotline Miami 2 and accidentally shot a meth lab, which exploded with a sound so loud I yanked my headphones off and sat there for a good minute waiting for my heartrate to slow.
