It's like somebody on the Xbox team was working on a horror game but by accident swapped the dashboard sounds with the sounds of his game
Back when I was about 8 years old, I was gifted my first PC with Metal Slug games in it.
I did not bother to start from the first game back then and just played the third game. I got to the second level and moved a bit, I saw zombies. Now, for person above the age of 10, pixelated zombies would be nothing to fear. But I was 8 and I just froze in place when I saw them, causing me to die. Excepting to re-spawn, I just waited.
Then I watched as my character became a zombie and got up. Shaking in my boots, I kept on walking until I saw a horde of zombies.
Then I tried to throw a bomb.

The Sandworms in one of the first missions of Star Wars Jedi Knight Jedi Academy freaked me the fuck out. Something about the notion of the moment you touch the sand that shit's coming for you, it freaked me out. I mean, it's not even that scary. You know it's coming and the ambient is bright, but it really freaked me out.
Sandworms enemies from the sphinx the curse of mummy completely caught me off guard
In the PC version Future Cop LAPD, there was a image for when you quit the game with a dude wearing some kind of helmet in front of a black background, and he says something in a low pitch voice. That had also freaked me out before.

It was intended to be "scary" in a sense that it's a really creepy boss but when i was younger this thing straight up haunted my dreams
Super mario 64, hazy maze cave, the lower part with the boulder and that loch ness monster looking thing you have to climb up.
To this day I have no idea why, but that thing freaked me the hell out. It's not hostile and largely ignores mario, yet every time I got to that part I had to hand the game to someone else or just stop playing it entirely. I was just so scared of it attacking me.
It's weird too because I replayed the game a few years after that and had no problem with that part, I couldn't even tell why I was scared in the first place. The thing is not designed to look scary at all.
The first game I ever played was called Timez Attack, kind of like an RPG where you did battles using math. About an hour in, you're pit against this guy:
