why not over-under barrel grenade launcher?

there's not really anything wrong with this weapon, but the recoil would kick something fierce IRL because of newton's law

Not really. Single barrel would kick about the same as an M203, which doesn't hurt at all so long as you keep the stock to your shoulder. It just pushes you back some, and it's not like you're worried about muzzle rise. Double barrel would be roughly the same, just on steroids. Remember, you've got the mass of the second barrel/firing mechanism to compensate for the added recoil, it's not going to be double the recoil.
It would also be heavy as fuck, because god-damn that is a lot of metal.

They exist. At least, things like them exist.
They're typically carried by entire gun-teams or mounted on vehicles though, being crew-served heavy weapons systems.
The Chinese do make an automatic grenade launcher that can be operated by just one person

Here's a QLZ-87 in use with Ugandan troops. It weights 26lbs/12kg in it's light bipod configuration and can be loaded with a 6 or 15 round drum magazine. There's also a newer version designated QLB-06

That's very close to what an M240B weighs. Whoever's going to be humping that in the field is in for a lot of pain, I'll say that much. Better hope he's friendly enough with the rest of his team that they'll help carry the ammo.
I'll always be a sucker for the Milkor MGL:

The problem I've always seen with heavier 'specialist' weapons like that grenade launcher and some others is that you don't get to carry a rifle of any sort into the field.

I've always wondered why the US never further explored the potential of a pump action grenade launcher. The Russians seem to like their GM-94.

They like using it in with HE/Thermobaric rounds. It sounds incredibly dangerous, but the explosion is relatively contained and doesn't produce shrapnel like a frag round. Also can be used to fire tear gas

The guys that came up with that grenade launcher also came up with the neat-looking and very-few-parts GSh-18.

I really want a GSh-18, don't think they export them though. Maybe someone in Ukraine can send one over.

They might seem exotic, but apparently they are not very pleasant to shoot and are of questionable quality. The triggers are heavy and inconsistent and the magazines are a pain in the ass to load, especially if you don't have the magazine loading device. In KardeN's example, the members of that unit had to file down the back of the frame for some reason, possible due to slide bite. The only thing that seems good about them is that they're light and can handle the high pressure 7N21/7N31 AP rounds. The Glock is also 2x more expensive than it in Russia and cannot handle those AP rounds.

Me and my pals were really into Far Cry 2's multiplayer on the 360. You could unlock the Milkor for one of the classes. Neat, right? Come to think of it, I can't think of a game that has had a better setlist of weapons than Far Cry 2 in recent memory. Anyways;
It was NOT neat. It was the most absolutely cheese fucking weapon I've ever encountered in a video game. It was just a sniper rifle that exploded like an RPG round. Lame as shit. Don't even get me started on it's rounds clipping through props and the terrain. I've developed some serious PTSD triggered just by seeing it. I hope that fucker holding it spent the rest of his deployment burning the shitty barrels they pull out of latrines.

I really like ArmA 3's Type 115. I'm not sure if it's based on any real weapon, but it's cool.

It fires 6.5mm, which is Arma 3's standard rifle ammunition, but it also has an underbarrel .50 for taking out whatever you want, whenever you want.

Honestly, so can any western 40mm, especially if it's swing-loaded like the European ones instead of the scrub-level M203. That way, you can load explosives of all types, smoke, gas, flares (Both parachute and regular), flechettes, less-lethal rubber batons and what-have-you-not. Pretty much anything you can fit in the barrel.
