I have a password for everything, however I use Firefox's "Password Storage" thing to remember them all for me. I'll probably start looking at my list, pick out some old good passwords, delete that site, reuse it for a different site or change my password to that.
Btw: The password has absolutely nothing to do with the website when I make 'em, is that a good idea? (and it's not something easy to guess either)
I use a random generated password with each account and keep track of all of them using KeePass and Dropbox. Ton more secure than using Firefox or the same password for each site, like I once did.
Have one for forums, one for email, and one for anything that involves money.
I have 3 passwords. 2 I cycle through on every website and another one for steam.
Steam password is much longer than the other 2, 32 characters long :v:

I recently got a dog, I couldn't think of a name for him so I called him Password.
Use an encrypted database program. They can randomly generate strong passwords and store them for you. I have about 40 different strong passwords that I would never remember otherwise. Also, make them at least 9 characters, to add a lot of time to cracking programs.
I tend to use the three first letters from the site and then my personal password after it.

Not that my passwords are actually that safe... I just memorize random sentences out of books and use parts of them (Rarely ever all of it because of character limitations), including punctuation and such.

Most of my passwords are variations of 1234 unless the passwords require at least 6 characters, the it is always 123456, unless there needs to be a mix of letters and numbers, then it is abc123.

Here's one of my password ideas, [email]yourpassword@siteyousignedupon.com[/email], assuming your password isn't lower capitals and different each time.
So if my password were "Giantd0ngs", i'd use "Giantd0ngs@facepunch.com" on FP, easy to remember and adds a bunch to the lenght.

sort of as easy to crack as a normal password, if I phished/other wise found out that your password for logging into facepunch was "password235@facepunch.com" I'd probably then try it on facebook "password235@facebook.com"

That's why I mentioned of having a different password each time, this "idea" just adds to the length of it.
Best rule:
Steam Password =/= other shit passwords
Three passwords too, steam and email are both important, other things not so much so.

All I do is go to a register or lost password page, then use the letters + words from the captcha image, and use it as my password.
I've been using a [url=http://www.passwordcard.org/]PasswordCard[/url]. Basically, a seeded sheet of random characters and letters you print out, keep the actual codes in your wallet and put the seed code somewhere really safe, and then just make passwords from those characters, and memorize them by the ASCII symbol and line color.
It's nice, because I don't have a stupid password for my GMail that I use for everything else.
