Seems kinda silly if David is exclusive to the server owner.

Not really, because he's not really meant to be an integral part of the game. If it was something like Hidden where someone got to be Dave each round, I don't think it'd be that fun. It would desensitize people to the character if someone played it poorly (i.e. just running around killing people, bunny hopping, talking to the other team, etc.)
Spike or Barley you should totally message me next time you guys go play, for some reason david is only on when you guys are, i would like to meet him, he sounds nice.
They lock the server so that people CAN'T join them and ruin it (rushing / micspam)

Believe me, you're not the only one. So believe me, when I say that David doesn't want to meet 1000 people, each person at a time.

The server is public and he sometimes goes on that server, he only put a password on it by the request of the guys who were playing, so they wouldn't get any random idiots that would join and just ruin the experience by just rushing trough the maps.

Yep this pretty much sums it up.
David showing up is a random chance, don't assume cause the server has a lot of people that he'll show up. If anything, he prefers smaller groups to prey on. Just keep playing, he'll show up eventually.

So, after doing a bit of searching, I've found a couple things.
He uses a modified version of the Metamod Eye module, which can be found [url=http://koti.mbnet.fi/axh/]on this page.[/url] But you can only set your own view to a player. If anybody has any idea how to edit Metamod modules, I'm sure people would appreciate it.
Any AMX/Metamod invisibility plugin will work for the disappearances.
I haven't really found anything else on the other stuff [Turning off lights, Killing with teamkilling disabled, Spawning chains of NPCS.] If anybody would like to help me in the search, I'd appreciate it.

This also gave me an idea of a game that would use the same system.
Kind of multiplayer horror-houses that players need to go through and then have a "director" or "omni-present admin" that would try to scare the shit out of the players..

Multiple people have expressed their desire to scare the crap out of people, why shouldn't we let them? I think opening a private server and inviting a few friends wouldn't hurt anybody.
Either way, I didn't mean to make that post to spoil his methods, or anything. I just want people to be able to scare their friends and such.

I think most of the stuff comes from the map and some sort of way to control the map itself via admin/console commands. Like the behind you and the lights thing.
