I find most of the songs mediocre, but a few like Blumenkranz, track 14, and I want to know to name a few are great ones.
It doesn't help either that some of the songs used to be overplayed as hell each episode (like lose your way for a prime example) which made the ost lose it's charm for me.

[sp] You cannot sever while striking the center as it's still a "rip" and will only heal itself. What Matoi needed to do is actually sever the limbs and attempt to sever her head, as shown with Ragyou even one small thread still connected can lead to fully recovery so the only actual way is for incineration of every life fibre gradually or slowly but surely fully severing the limbs.
Remember, Ragyou also healed fully even though pierced in the chest, so it's highly likely that the Heart is extra-durable. [/sp]

Depends on if they're actually dead or not... We've never seen one die, and considering they ARE Honnouji Academy students, they're probably still all alive.
They were eaten by their clothing, but they can be saved pretty easily, if Ryuuko's tantrums are any indication.
So... Nobody's died aside from Isshin, that we know.
