I dunno about Standard grades but I know our Highers are harder than A levels. I got told for A level English you are allowed to take the book into the exam with you, and read it (for quotes) during the exam.

I got an X last year in IT, turns out someone who shared the same name (first not last) as me had dropped it and so somehow his grade, or lack thereof had got tacked onto mine too.

Depends, you can either enter at foundation tier - in which the highest possible mark is a C - or higher tier, where the highest mark is an A* (which is the highest possible grade for any GCSE. The pass boundaries are also different.
I think there's intermediate tier in some subjects as well, I don't know that much though.
