Since Kim is in bad health, keeping the current ruling party in control is top priority in North Korea right now. His successor is believed to be planned as little more than a figurehead, having one of Kim's most trusted buddies making the real decisions. The people don't give a crap about how little they have. They have no way of knowing what life is like outside of North Korea, and even if they find a tiny bit of evidence suggesting that life is better elsewhere, every other source they've ever encountered will suggest otherwise, so they'll almost definitely think it's a lie. They probably think the outside world is barren wasteland filled with barbarians who are jealous of how much North Korea has. No revolution is happening within anytime soon.

Castro said that if anyone wanted to leave Cuba they could, approximately 1.4 million cubans left at their own accord and were labeled as exiles for not staying with Castro. It may not be everyone but it's still alot.
