The terrorist had bunch of shells next to him. The mortar was probably buildt from scraps and blew into pieces, and the explosion caused rest of the shells to blow up.

The worst sceneario is the pin coming off while it's still in your grenade pocket, And the grenade pockets are generally made so that even if the pin would come off the other parts would stay inplace -preventing the fuse from being lited.

the reason he blew up in the end was a new spotting system was being used against him it pin points the origin of the enemy artillary and fires off a round to counter it
