It would be good if we could think of a way to stop the double banning happening, though it doesn't happen all that often. When we could see who was in a thread that helped, as I could also see who else was looking at a report.

How about: You can act upon your own ban (if you mess up etc.), but other moderators are blocked from acting upon the same post for 5 minutes or so.

I was thinking the page checks if the user is banned when you submit the action and displays a confirmation 'user is already banned - proceed anyway?' but getting garry to change things is tough
