I had the same thing. Either don't use a condom (because your girl's on the pill, and you trust her) or use a bigger condom.
She is on the Pill, I trust her, but she is forgetful, and has taken it late in the past, so would rather stick to condoms, since we're both 17 living at home. Bigger condoms I will try I think.

Its happened 3 times out of the past 4. The last time it didn't because we had to stop because her hips were hurting.
1. either not putting it on right, i.e leaving enough room at the tip, pressing it to remove air bubbles.
2. condoms are expired. maybe put in a hot place, causing mini tears in the rubber.
3. there might be not enough natural lubrication, and adding some water based lubrication might be essential.
4. condoms are too small, too big for ya.
5. you might be opening the wrapper in a way that damages the condom
1. I assure you I'm not, I always hold the tip to stop air getting trapped and roll them on. Plus the durex ones have a little bit in the end made into them so you don't have to leave a bit.
2. I always check this too, they are all 2013 expiry dates. They're kept in the box in my cupboard so not getting hot..
