Children can look feminine, in stature... MGSV takes place in 1984 (right after Ground Zeroes which takes place 9 years after Peace Walker [1974]), The first MGS takes place in 2005... That's 21 years... If Psycho Mantis was a (let's say, judging by that picture) 6-7 year old kid, that would make him 27-28 during MGS1 (which would be a good age for any of the criminal cronies you see throughout the series, except for The End lol). I'm like 95.5% sure that that is Psycho Mantis, as a child.

This makes me incredibly excited for MGSV, but also so sad that this incredible series has come to an end.
