My biggest fear as a kid was that there would be a spider in the toilet and I wouldn't notice until it was too late...until it crawled up inside me.

Why don't you just step on them, or kick them to the side, or even pick them up and put them in the wash (or yell at them to do that). You aren't going to get a disease from stepping on used clothing. I would imagine they would have to be quite unhygienic for you to have any effect at all from stepping on them, even still worst thing I can think of is that you get a lingering smell or urine/feces/semen on your foot, but judging by the age I highly doubt they would be leaving any of those in their underpants.
